Illinois Residents

  1. Gather your documents:

    • Driver's License/ State ID
    • 2 pieces of mail from your current address
    • Benefit Verification Letter/ DD214 Form
    • Recent Medical Records
      (We also diagnose conditions.)

  2. Schedule your visit:

    • Call us at (815) 208-7703 or click here to make your appointment with our cannabis friendly doctor.

  3. Submit your application:

    • After seeing our cannabis-friendly doctor, we will submit your application free of charge. Within 48 hours, you will be able to go to the dispensary of your choosing.

Iowa Residents

  1. Gather your documents:

    • Driver's License/ State ID
    • Recent Medical Records
      (We also diagnose conditions.)
    • Reduced fee documents:
      Copy of SSI/SSDI Notice, Medicaid card, Hawk-I member card

  2. Schedule your visit:

  3. Submit your application:

    • After seeing our cannabis-friendly doctor, we will send you your health care practitioner form along with instructions on how to complete your application online.
